Amenity Card Registration

For Amenity Cards, please fill out the form below (one form = one card).  

*At this time, we will assign up to two cards per address, by CDD Board direction.*

Once completed and submitted, this form is then placed in a queue for review and confirmation of residency and qualification.

An email will be sent to the email you provide below (in the order in which it is recieved) to confirm receipt and as a follow up to include a link for submitting your payment electonically.

NOTE: We now have a faster online payment option that will charge a service fee for credit card payment.

Check payments will remain an option, though subject to additional wait time for confirmation and processing. PLEASE DO NOT SEND A CHECK UNTIL YOU RECEIVE CONFIRMATION.  Sending a check before confirmation will NOT speed up the process and may result in delays if a refund is due to you.
